Background Check Authorization

I hereby authorize etools4Education, LLC and its designated agents and representatives to conduct a comprehensive review of my background causing a consumer report and/or an investigative consumer report to be generated for employment purposes. I understand that the scope of the consumer report/investigative consumer report may include, but is not limited to, the following areas:

• Verification of Social Security Number
• Current and previous residences
• Employment history including all personnel files
• Education including transcripts
• References
• Credit history and reports
• Criminal history records from any criminal justice agency in any or all federal, state, county jurisdictions
• Birth records
• Any other public records

I, __________________________________, authorize the complete release of these records or data pertaining to me which an individual, company, firm, corporation, or public agency may have. I understand that I must provide my date of birth to adequately complete said screening and acknowledge that my date of birth will not affect any hiring decisions. I hereby authorize and request any present or former employer, school, police department, financial institution or other persons having personal knowledge of me to furnish etools4Education, LLC or its designated agents with any and all information in their possession regarding me in connection with an application of employment. I am authorizing that a photocopy of this authorization be accepted with the same authority as the original.

I hereby release etools4Education, LLC and its agents, officials, representatives, or assigned agencies, including officers, employees, or related personnel both individually and collectively, from any and all liability for damages of whatever kind, which may, at any time, result to me, my heirs, family, or associates because of compliance with this authorization and request to release. You may contact me as indicated below for purposes of completing this investigation.

Fair Credit Reporting Act Disclosure

In accordance with the Fair Credit Reporting Act (FCRA, Public Law 91-508, Title VI), this information may be provided to credit agencies, as well as to others who may legally receive it. Pursuant to California Civil Code, I further understand that etools4Education, LLC will access public and private information to ascertain information for employment purposes. I understand that I have the right to request from etools4Education, LLC, upon proper identification, the nature and substance of all information in its files on me at the time of the request.

Confidentiality Agreement

If employed by etools4Education, LLC, I understand that I will have access to sensitive and private information from teachers in the alternative certification program. I agree to maintain strict confidentiality of all student, teacher, and program information. I understand that misuse or disclosure of confidential information may result in disciplinary action up to and including termination of employment and possible legal action.

Applicant's Statement

I certify that the information provided in this employment application is true and complete to the best of my knowledge. I understand that any false information, omissions, or misrepresentations of facts called for in this application may result in rejection of my application or immediate discharge at any time during my employment.

I authorize etools4Education, LLC to verify any or all of the information provided on this application, including contacting current or former employers, educational institutions, and references. I authorize all persons, schools, current and former employers, and law enforcement authorities to release any information concerning my background that may be relevant to evaluation of this employment application and I hereby release any such persons, schools, employers, and law enforcement authorities from any liability for damages whatsoever for issuing this information to etools4Education, LLC or its agents.

I understand that employment with etools4Education, LLC is "at will," which means that either I or etools4Education, LLC can terminate the employment relationship at any time, with or without prior notice, and for any reason not prohibited by law.
